The following are frequently asked questions regarding your visit.
WHAT DO I BRING TO MY VISIT?
For your first visit, please bring the following items:
1. Your completed "PATIENT REGISTRATION FORM' and "MEDICAL HISTORY FORM"
These can be downloaded by clicking the "Our Practice" tab, then click the " Forms For Your Visit";
2. Your insurance card(s);
3. All medications and supplements you are currently taking
This is very important to ensure we understand exactly what you are taking at the time of office visit.
For your follow-up visits please bring:
1. Your insurance card(s);
2. All medications and supplements you are currently taking.
DO YOU FILE INSURANCE?
Our office does not file paperwork for insurance claims.
You will be given a completed “super bill” (and a duplicate copy for your records) with all the codes necessary for you to file a claim with your insurance carrier.
A copy of your medical records will be forwarded to the insurance company when a signed authorization to release medical records is received. Their medical review professionals can extract the information required for your claim from these records.

Once you enter the building please go the the 3rd floor and proceed to the end of the hallway. The name of each practice is displayed on an overhead hanging plaque outside each office.
At the end of the hall you will come to the "Executive Office" suite. If you look up, you will see "LMDI" on a hanging plaque. Please walk into the Executive Office suite. Our office is the first door on the left.
